Eligibility for Inclusion Support

The Child Care Service (CCS) may be eligible provided that the child for whom the funding support is required is from one of three eligibility groups:

  • a child with a diagnosed disability,
  • a child undergoing continuing assessment for a disability,
  • or a humanitarian refugee child who demonstrates behaviours symptomatic of having experienced torture or trauma.

Documentary evidence for child eligibility may include:

  • A Doctor's Diagnostic Report Form. This report must have been signed within the last 12 months
  • Centrelink documentation stating a parent/s or guardian/s eligibility or approval for Carer's Allowance with respect to the child or evidence of a Health Care Card in the name of the child, marked with the disability code CD or DSP.  An appointment letter for an assessment by a medical practitioner
  • Documentary evidence of the child's humanitarian refugee status as identified by appropriate visa codes.

Please note: Due to guidelines changes this will need to be verified at the time that the Service Support Plan and the Inclusion Support Subsidy Application are being developed.
